Initial visit
 

At your initial visit, you will fill out an intake form about your condition. You can download this form from our website and bring it for your initial consultation (‘Patient Information Form’). The nurse will then review your history, and go over any existing imaging studies, such as X-Rays and/or MRIs. You may need to have more X-Rays taken at this time to better diagnose your condition. The physician will then perform a thorough and comprehensive physical exam and take a detailed history of your condition. He will also review your imaging studies. After this, your physician would go over his findings with you and address any questions/ concerns that you might have.
